The Gentle Departure of Mai: Finding Peace and Dignity in Home-Based Care

For eighty-four years, the woman affectionately known as “Mai” led a life defined by diligence and devotion. A retired bank professional from Moshi, she was a person of sharp intellect and deep-rooted struggles, having successfully navigated a long career while raising sons who eventually flourished in the IT and private sectors. However, five years ago, a diagnosis of breast cancer introduced a new kind of struggle. Despite receiving curative treatment at Sahyadri Hospital, the progression of the illness eventually led the medical team tending to her to a difficult conclusion: the time for aggressive intervention had passed, and the time for a compassionate, palliative approach at home had begun. When CanSupport registered Mai in April 2025, she was still mobile with a walker, but as the months transitioned into the winter of her life, her strength faded, and she became completely bedridden.

This physical decline brought with it a complex emotional landscape. Mai, who had always been emotionally stable, began to experience the frustrating cognitive fog that often accompanies advanced illness. She struggled to find her words, leading to sudden irritability and outbursts of anger toward her loved ones. The daughter-in-law, who served as the primary caregiver, bore the brunt of this emotional storm, finding herself exhausted by the constant care and the weight of Mai’s negative thoughts about the end of life. The challenge for the CanSupport team was not just to manage Mai’s physical comfort, but to mend the fraying emotional threads within the household, ensuring that the final chapter of her life was defined by love rather than the stress of the disease.

The intervention focused on building a bridge between Mai’s past and her present through supportive counseling and active listening. During home visits, the team encouraged Mai to share her “treasures” – memories of her banking days, the pride she felt in her sons’ upbringing, and her lifelong love for reading. By honoring her as “Mai” rather than just a patient, the team established a rapport that transcended the medical, making her feel like a respected elder rather than a burden. Simultaneously, separate counseling sessions were held for her sons and daughter-in-law. The team guided them to understand that her anger and communication difficulties were symptoms of her condition, not reflections of her heart. This insight empowered the family to respond with patience, focusing on fulfilling her small daily wishes to ensure she felt emotionally satisfied and deeply respected.

As Mai’s journey neared its end, the nursing team provided the practical scaffolding needed to support the family’s devotion. They educated the household on the essential care of a bedridden patient which entails meticulous hygiene, careful positioning to prevent complications, and proper medication administration. This holistic support ensured that Mai remained comfortable in her own bed, surrounded by the people she loved. When the time came to prepare for the inevitable, the team provided sensitive end-of-life counseling, helping the family navigate the profound transition ahead with clarity and courage.

On a February morning, Mai passed away peacefully in the home she loved, having achieved the “good death” that palliative care strives to provide. Even in the immediate aftermath, the CanSupport team remained a constant presence, guiding the family through the necessary administrative procedures and providing the emotional support needed to begin their grieving process. The family expressed profound gratitude, noting that the team never felt like outsiders, but like extended members of their own family. Through this dedicated journey of care, Mai was able to leave the world not in a clinical ward, but in a place of dignity, where her stories were heard, her wishes were met, and her life was celebrated until the very last breath.
